How can I sync my iphone to itunes without it messing up my playlists?

Q. Well I've been using Iphone 4 since it came out. The first one I had needed to be replaced because of problems with the phone. When I got a new one the took the memory card from my old phone and put it in the new one. I noticed all my apps were gone but I got them back on iCloud. When I downloaded the apps again I noticed all my memory was gone from them. For example, my music app called "Mp3 Skull" all my music was deleted even after I recovered my account. Is there any way to not let this happen If I decided to upgrade to an Iphone 5?
